01Contact us
hello@formcheckco.app
We aim to reply within 2 business days. Reports of objectionable content or of anything involving a young person's safety are actioned within 24 hours and jump the queue — put URGENT in the subject line.
What to put in the email
- What you were doing, and what happened instead
- Your phone and iOS or Android version, and the FormCheck version from the bottom of the Settings screen
- If it is about a specific kick: which measure looked wrong. That is usually enough to find it
Do not send us a clip unless we ask for one. FormCheck never receives your video and we would rather keep it that way — especially if the person in it is under 18. Almost everything is diagnosable from a description.
02The app
It says it could not read my kick.+
That is the app refusing to guess, and it is the app working. It needs a clean side-on view of the whole body. Film from the side, about ten metres back, phone upright, whole body in frame. Filmed from behind, the final step cannot be seen at all, so there is nothing to measure.
A measure says NOT MEASURED.+
It could not see that part of the body clearly enough through the kick. A confident wrong number is the failure that actually costs you, so it says so instead. Re-film with the whole body in frame and better light.
My score went down.+
Scores are compared to your own baseline — the average of your first three readable kicks — not to a professional. One kick is noise. The app waits for a pattern across sessions before it changes what it tells you to work on, and you should too.
Why does it only give me one thing to work on?+
Because a list of six corrections puts you back where you started. A focus stays until it improves and holds across two sessions, then it retires and the app picks the next one.
The Coach says it is not switched on in this build.+
The open-ended half of the Coach needs a connection to an AI provider. Everything about your own numbers, what to work on, how the app measures a kick, and building a session still works offline and for free — try one of the suggestion chips.
Where is the Community tab?+
It is not switched on. It is hidden rather than shown as an empty room, because a feed nobody can post into is worse than no feed. See the privacy policy for what it will do and the rules it is built on.
Nothing is coming through from Apple Health.+
iOS deliberately will not tell an app whether Health read access was granted, so the app cannot tell "you said no" from "there is no data". Check iOS Settings → Health → Data Access & Devices → FormCheck and switch on sleep, resting heart rate and workouts. If your watch or ring has not written anything into Health yet, there is genuinely nothing to read.
Does it work for rugby league?+
Yes. A goal kick off a tee is the same movement in both codes, and the research the app is built on is place kicking.

04Your data and deletion
Everything is on your phone. Your clips, scores, athletes and logged sets never leave it, so there is no account for us to look up and nothing for us to export to you — it is all already in front of you in the app.
To delete everything
Settings → Erase everything, then confirm. That removes every kick, every athlete, every logged set, your account record, your saved Coach conversation, the usage log and anything the community feature stored. Deleting the app does the same.
To see what the app records about its own use
Settings → Things recorded lists every entry. The row underneath erases just that log and leaves your kicks alone.
To disconnect Apple Health
iOS Settings → Health → Data Access & Devices → FormCheck. Only iOS can revoke it. The app stores nothing it reads from Health.
Full detail on all of this is in the privacy policy. Privacy questions and complaints go to hello@formcheckco.app.
05Reporting content or abuse
If a young person is at risk
Email hello@formcheckco.app with URGENT in the subject. We action it within 24 hours. If someone is in immediate danger, contact the police first — in Australia, 000.
Inside the app, every post and comment can be reported and every user can be blocked. A report hides the content from you immediately.
You do not need an account to report something to us, and you can report on behalf of someone else — a parent, a coach, a school. Tell us what you saw, where, and roughly when. We remove objectionable content and terminate the accounts behind it. See the terms for what counts and what we do about it.
06Club codes and funded places
Clubs that cannot pay do not. If yours needs a place, email hello@formcheckco.app with the club or programme's name and who you are.
- Places are finite and the app shows how many are currently funded. We will not pretend otherwise.
- A funded place is the whole app, not a trimmed version.
- Enter the code on the sign-in screen, or in the app under the club-code option.
- "That code is not one of ours" means it is mistyped or was not issued by us. Check it with whoever gave it to you.
07Billing
Nobody is being charged for FormCheck right now. There is no way to buy anything in the current release, and the coaching features are open to everyone.
When subscriptions are switched on, Apple and Google take the payment, not us. That means we cannot see your subscription, cannot cancel it and cannot refund it.
- Cancel — iPhone: Settings → your name → Subscriptions → FormCheck → Cancel.
- Cancel — Android: Play Store → profile → Payments & subscriptions → Subscriptions.
- Refunds: request through Apple (reportaproblem.apple.com) or Google Play. If you think something went wrong on our side, email us and we will help you push it.
Deleting the app does not cancel a subscription. Your rights under the Australian Consumer Law are not affected by any of this — see the terms.
08Accessibility
Interactive controls carry VoiceOver labels, every animation in the app honours Reduce Motion, and every text-and-surface colour pairing is checked against WCAG AA contrast as part of the build rather than by eye.
If something in the app is unusable for you, tell us what and on which screen. We treat that as a bug, not a feature request.
09Reporting a bug
Send it to hello@formcheckco.app with the screen it happened on, what you expected, and what you got. A screenshot of the screen helps. A clip of a young person does not — describe it instead.
If the app produced a number you think is wrong, say which measure and what you believe the truth was. A wrong number that looks right is the single failure this app is built to avoid, so those reports get looked at first.
