01The agreement
These terms are the agreement between you and FormCheck ("FormCheck", "we", "us"), an independent Australian project operated from Sydney, New South Wales, for your use of the FormCheck mobile app and the website at formcheckco.app.
By downloading, installing or using the app you accept these terms. If you do not accept them, delete the app. These terms and our privacy policy are the whole agreement between us about FormCheck.
This document is also the end-user licence agreement (EULA) for the app. Where you obtained the app from the App Store, it replaces Apple's standard licensed application end user licence agreement, subject to section 16.
02Who may use it
FormCheck is built for rugby kickers, coaches and families. Many of the people who use it are school-age.
- If you are under 18, you may use FormCheck with the knowledge and permission of a parent, guardian, coach or teacher. By using it you confirm you have it.
- If you are filming someone else — a coach filming a squad, a parent filming their child, an athlete filming a mate — you are responsible for having that person's permission, and their parent or guardian's permission if they are under 18, before you film them.
- If you are a coach or a club using FormCheck with young players, you are responsible for your own duty of care, your own child-safety obligations and your own club's policies. FormCheck does not replace any of them.
The app asks for no age and cannot verify one, which is exactly why it collects no personal information and holds no account. See the privacy policy.
03Your licence
We grant you a personal, limited, non-exclusive, non-transferable, revocable licence to install and use one copy of FormCheck on devices you own or control, for your own non-commercial use and — if you are a coach or club — for coaching the athletes in your care.
You may not sell, sublicense, rent or redistribute the app; copy or modify it; reverse engineer, decompile or disassemble it except to the extent that law expressly permits despite this limitation; strip out or obscure any notice in it; or use it to build a competing product.
Where you obtained the app from the App Store, this licence is limited to use on Apple-branded products that you own or control, and as permitted by the Usage Rules in the Apple Media Services Terms and Conditions.
04Not medical advice
Read this one properly
FormCheck is not a medical device. It does not diagnose, treat, cure or monitor any injury, illness or condition. It is not a substitute for a physiotherapist, doctor, sports physician or any other qualified professional, and nothing in it is medical advice.
FormCheck does not predict injury and makes no claim about your individual risk. The app deliberately does not carry a "readiness" score, an injury-risk figure or a workload ratio, because the published evidence for those does not support what products built on them claim.
If something hurts, if a joint gives way, if there is swelling, or if you are worried — stop and see a professional today. Not tomorrow, and not after one more set.
The prevention exercises, stated exactly
FormCheck prescribes two conditioning protocols — the Nordic hamstring exercise and Copenhagen adduction — and states the published trial results for each, with the dose the evidence used.
Those results are group-level findings from randomised trials in team sport. They describe what happened to injury rates across a squad over a season. They are not a claim about you, they are not a promise that you will not get injured, and doing the work does not make you safe. The app is written to say this and the AI coach is blocked from saying otherwise.
Do not start either protocol, or any drill in the app, if you are injured, in pain, or coming back from an injury, without clearing it with the person treating you.
05What the numbers are
FormCheck estimates body position from a single 2D phone camera. Camera angle, distance, lighting, loose clothing and anything blocking the body all move the numbers. The moment of ball contact is inferred from the movement, not measured by a sensor in the ball.
The scores are 0–100 figures computed against published research bands and against your own baseline — the average of your first three readable kicks. They are a coach's eye, not laboratory biomechanics, and they are most useful as a pattern across sessions rather than as a verdict on one kick.
The app is built to refuse rather than guess: if it cannot track a body cleanly it says so and tells you how to film it better, and if it cannot see one part of the body it marks that measure as not measured instead of inventing a value. A refusal is the app working, not the app failing.

06The AI coach
The Coach tab uses a large language model. Most questions are answered on the phone from your own stored numbers; open-ended questions are sent to a third-party AI provider. What is sent is set out in the privacy policy.
The Coach is deliberately constrained: it is given the app's published research figures and your own scores and is instructed never to invent a norm, and its replies are then checked — a reply quoting a measurement the app has not measured, or straying into pain, injury or diagnosis, is discarded rather than shown to you.
Those controls reduce the risk; they do not remove it. A language model can still be wrong. Treat what the Coach says as a starting point for a conversation with a real coach, never as instruction to follow blindly, and never as clinical advice.
Do not type personal information, anyone's name, or a description of an injury into the Coach. Anything you type is sent to the provider.
07Acceptable use
You agree not to:
- Film anyone without their permission, or a young person without their parent's or guardian's permission
- Use FormCheck to assess, rank, select, drop or discipline another person in a way that affects them, or to present its output as a medical, clinical or fitness-to-play opinion
- Present a FormCheck score as a professional assessment, or misrepresent what the app measured
- Use it to harass, bully, shame or intimidate anyone
- Interfere with the app, attempt to defeat its safety checks, or use it to build or train another product
- Use it in any way that breaks the law, or in breach of a club, school or governing body policy that applies to you
08Content, and no tolerance
Community feature not currently switched onFormCheck includes a community feature — a group feed for a club or school — that is not active in the current release. These rules apply in full from the moment it is, and to any content you send us at any time.
There is no tolerance for objectionable content or abusive behaviour
This is not a warning, it is the policy. Posting objectionable content or abusing another user results in the content being removed and the account being terminated, and you may not create another one.
What is objectionable
Anything sexual, sexualised or involving a minor in any sexual context; nudity; violence or gore; threats; harassment, bullying or intimidation; hate speech or abuse based on race, ethnicity, religion, disability, sex, gender identity or sexual orientation; self-harm or suicide content; illegal activity; drugs, alcohol or gambling promotion; impersonation; spam, scams or commercial solicitation; anyone's personal contact details; doxxing; and any content posted without the consent of the people in it.
What we do about it
- Filtering. Every piece of text you write is screened before it can be posted. Contact details of any kind — phone numbers, emails, links, social handles, "add me on" — are refused outright, in every field.
- Review before publication. Posts and comments are not visible to anyone else until they have been reviewed.
- Reporting. Every post and comment can be reported from the app. A report hides it from you immediately, and a small number of independent reports removes it for everyone pending review.
- Blocking. You can block any user. Their content is then never sent to your device, and yours is never sent to theirs.
- Ejection. We remove content and terminate accounts for objectionable content or abusive behaviour, at our discretion, without refund.
- A response time. We act on reports of objectionable content within 24 hours of receiving them.
- A published contact. hello@formcheckco.app, monitored by a person.
No private messaging
FormCheck has no direct messages and no private one-to-one channel of any kind, by design. Everything anyone writes is attached to something visible to the whole group. Attempting to move a conversation off the app is a breach of these terms.
Your content
You keep ownership of anything you post. You grant us a non-exclusive, worldwide, royalty-free licence to host, store, reproduce and display it for the purpose of running the feature, and to keep a copy of removed content where we need it to handle a report or meet a legal obligation. That licence ends when you delete the content, except for those two purposes.
You must have the right to post what you post, including the permission of everyone appearing in a clip and, where they are under 18, their parent or guardian.
09Funded and free places
FormCheck funds places for clubs and programmes that cannot pay, including First Nations rugby programmes. A funded place is the whole app, not a cut-down version — a kid at a community club getting a lesser product than an academy kid would defeat the point of it.
- Places are finite and the app says how many are currently funded. We do not promise unlimited free access, because that is a promise nobody can keep.
- A code is issued to a club or programme by a person, not generated on a website, and is redeemed inside the app. Redemption happens on the device, so nothing about it reaches us and we cannot count or track it.
- A code is not transferable, has no cash value, cannot be exchanged for money, and grants access for the period stated when it is issued.
- Because redemption is offline, a code cannot be revoked once redeemed. That is a deliberate trade-off for not putting a minor's data on a server.
- We may decline to issue a code, and we may stop issuing them, at our discretion.
Clubs that need one should email hello@formcheckco.app with the club's name and who you are.
10Subscriptions and payment
Nothing is charged todayThere is currently no way to pay for FormCheck and nobody is being charged. Until payments are switched on, the coaching features are open to everyone and the app says so on every screen that mentions them. This section describes what applies once they are.

A paid feature never makes a free read worse. No blurred scores, no hidden measurements, no watermark, no reduced accuracy. If the camera saw it, you get it.
Price and billing
The intended price is AUD $12.99 per month or AUD $89 per year, inclusive of GST. Prices are shown in the app before you buy and may change; a change never applies to a period you have already paid for.
Subscriptions are sold and billed by Apple through the App Store or by Google through Google Play, not by us. Payment is charged to your store account at confirmation of purchase. A subscription renews automatically unless you turn off auto-renew at least 24 hours before the end of the current period, and your account is charged for renewal within 24 hours before the period ends.
You manage and cancel your subscription in your own account settings — App Store: Settings → your name → Subscriptions; Google Play: Play Store → Subscriptions. We cannot cancel it for you and we cannot see it. Deleting the app does not cancel a subscription.
Refunds
Refunds are handled by Apple or Google under their own policies, because they took the payment. Ask them first. Nothing here limits your rights under the Australian Consumer Law — see section 12.
11Ownership
FormCheck, its software, its design, its scoring method, its wording and its name are ours or our licensors'. These terms grant you a licence to use the app, not any ownership of it.
Your kicks, your clips, your scores and your history are yours. They sit on your phone, we never receive them, and we claim nothing in them.
The research FormCheck cites belongs to the researchers who did it. The app names them and links to the source rather than presenting their findings as its own.
FormCheck is not affiliated with, endorsed by or sponsored by any club, school, league, code or governing body.
12Warranties and liability
Australian Consumer Law
Our goods and services come with guarantees that cannot be excluded under the Australian Consumer Law. Nothing in these terms excludes, restricts or modifies any right or remedy you have under it, or under any other law that cannot be excluded by agreement. Everything below applies only so far as the law allows.
Beyond those guarantees, FormCheck is provided "as is". We do not warrant that it will be uninterrupted or error-free, that a reading will be accurate for any particular clip, or that using it will improve your kicking or protect you from anything.
To the extent the law allows, we are not liable for indirect or consequential loss, loss of opportunity, or any injury, aggravation of an injury, or health outcome arising from your use of FormCheck or from anything you did because of what it told you — including anything said by the AI coach. Where our liability cannot be excluded, it is limited, at our option, to re-supplying the service or refunding what you paid us for it in the twelve months before the claim.
You are responsible for your own body, your own training decisions and your own duty of care to anyone you coach.
13Termination
You can end this agreement at any time by deleting the app. That erases everything it holds — see the privacy policy. Cancel any subscription separately, in your store account.
We may suspend or terminate your access, remove content, and refuse to issue or honour a code if you breach these terms — immediately and without refund in the case of objectionable content, abuse of another user, or anything that puts a young person at risk.
We may also discontinue FormCheck, or any part of it. If we do, we will say so on this site, and because your data is on your phone rather than on our server, nothing of yours disappears with us.
Sections 4, 5, 11, 12 and 15 survive termination.
14Changes
We may update these terms. The version and effective date at the top of this page always say which version is current. A material change — the community feature going live, payments switching on, a change to what leaves your device — will be called out in the app or on this site rather than slipped in. Continuing to use FormCheck after a change means you accept it; if you do not, delete the app.
15Governing law
These terms are governed by the laws of New South Wales, Australia, and you and we submit to the non-exclusive jurisdiction of the courts of New South Wales and the courts able to hear appeals from them. If you are a consumer somewhere else, this does not take away any protection your local law gives you that cannot be excluded by agreement.
If any part of these terms is unenforceable, it is severed and the rest keeps working.
16Apple & Google terms
If you obtained FormCheck from the App Store, the following applies and prevails over anything inconsistent above:
- This agreement is between you and FormCheck only, not Apple. FormCheck, not Apple, is solely responsible for the app and its content.
- Your licence to use the app is limited to a non-transferable licence to use it on any Apple-branded products that you own or control, as permitted by the Usage Rules in the Apple Media Services Terms and Conditions.
- Apple has no obligation to furnish any maintenance or support for the app. Support is ours, at formcheckco.app/support.
- In the event of any failure of the app to conform to any applicable warranty, you may notify Apple, and Apple will refund the purchase price (if any) to you. To the maximum extent permitted by law, Apple has no other warranty obligation whatsoever with respect to the app.
- FormCheck, not Apple, is responsible for addressing any claims by you or a third party relating to the app or your use of it, including product liability claims, any claim that the app fails to conform to a legal or regulatory requirement, and claims arising under consumer protection, privacy or similar legislation.
- In the event of a third-party claim that the app or your use of it infringes that party's intellectual property rights, FormCheck, not Apple, is solely responsible for the investigation, defence, settlement and discharge of that claim.
- You represent and warrant that you are not located in a country subject to a U.S. Government embargo or designated as a "terrorist supporting" country, and that you are not listed on any U.S. Government list of prohibited or restricted parties.
- You must comply with any applicable third-party terms of agreement when using the app.
- Apple and Apple's subsidiaries are third-party beneficiaries of this agreement, and upon your acceptance of these terms Apple has the right (and is deemed to have accepted the right) to enforce them against you as a third-party beneficiary.
- Our contact details for questions, complaints and claims are in section 17.
If you obtained FormCheck from Google Play, the Google Play Terms of Service apply to your download and any purchase, and Google is not a party to this agreement.
